This request applies solely to personal information of California residents (collectively, "you", "your") processed by United Heritage Financial Group, United Heritage Life Insurance Company, United Heritage Property & Casualty Company, and Sublimity Insurance Company (collectively, "United Heritage," "we", "us", or "our") to the extent the personal information is subject to the California Consumer Privacy Act and California Privacy Rights Act (collectively, the "CCPA").
Only the California resident or someone legally authorized to act on the resident’s behalf may make a verifiable consumer request related to the personal information. You may designate an authorized agent by using our Authorized Agent Affidavit form or by providing us with a copy of an appropriate power of attorney. A parent or guardian may also make a verifiable consumer request on behalf of a minor child (a consumer who is less than 13 years of age).
After we process your form, we will contact you if we need to collect additional information from you in order to properly verify the request. Only once we have verified the identity of the consumer and the authority of the requestor to make the request will we process your request.
