• Complete the CCPA Request Form

To designate an Authorized Agent to make a CCPA request on your behalf:

• Call 1-800-657-6351 ext. 4000 to have a representative either mail or securely email the affidavit form to you
OR
• Download the Authorized Agent Affidavit Form to complete and mail to us
